1) Your name 

2) Student number 

3) Current city and state 

4) Course number and professor's name 

5) Course type (online, hybrid, extension center, and weekly intensive) 

6) Assignment type and due date.

 

Also, be sure to add any specific questions or topics with which you would like help or feedback. If any of this information is lacking, it will be requested.
